Privacy Policy

Our Commitment to Privacy: The YesUtah Donor Registry is operated by DonorConnect, the federally designated, nonprofit community service organization dedicated to the recovery and transplantation of organs and tissues for Utah, southeastern Idaho, western Wyoming, and Elko, Nevada. DonorConnect partners with Utah Lions Eye Bank, a nonprofit organization, that helps facilitate the gift of sight through cornea and eye donation. DonorConnect is committed to protecting the privacy of any personal information provided by site users, along with the security of information entrusted to the registry. The process outlined below details DonorConnect’s privacy position.

Policy and Procedure:

  1. Requirements
    1. 1.1.     Donor Registry: In order to create an account and register as a donor, we require the following personal information: first and last name, street address, city, state, zip code, email address, and date of birth. You can access your profile by entering your registration id or username and password each time you use the site.
    1. 1.2.     Financial Contributions: If you choose to make a financial contribution, we require your contact information and collect standard credit card information (card number, card type, expiration date) and keep a record of your donation. Credit card numbers are held only until the charge can be processed (usually several minutes), and then the number is only available for purposes of problem resolution.
    1. 1.3.     Use of Cookies: During your visit to the YesUtah or YesIdaho Donor Registry, a temporary “cookie” is stored on your computer’s hard drive. Cookies do not capture any personal information, however they are used for authentication, tracking and speeding up access to information you may want to view. When you log off the site, the session-based cookie is automatically cleared from your system. Most web browsers accept cookies, and your browser’s “Help” menu should provide options for the manipulation (i.e. preventing, accepting, or receiving notice) of new cookies. Please be aware the blocking of cookies may result in some site functionality being lost.
  2. Information We Collect
    1. 2.1.     Standard Web Server Traffic Pattern Information: General traffic, site usage, and length of stay information is collected and stored in log files. Log files are transaction records maintained on Web servers to record information such as: Service provider IP addresses, browser versions, referring websites, average number of pages requested, average duration of visit to the Site, and total visitor traffic. This type of information is shared externally only on an aggregated basis.
    1. 2.2.     Personal Information: We do not collect personally identifiable information from you unless you provide it to us voluntarily and knowingly, as described below.
  3. How Your Information is Used
    1. 3.1.     Donor Information: Personal identifiable information submitted by site visitors wishing to be an organ, eye or tissue donor will be added to the Registry and kept confidential. Use of donor information is restricted to those purposes set forth in applicable state donor laws. Specifically, your information is only used in matching your organ donor designation with organ and tissue recovery agencies looking for potential donors. Your information is only shared with organizations that are authorized by the YesUtah or YesIdaho Donor Registry, and the organ procurement organization (DonorConnect) to verify this information in the case of a death of a registered donor. Your donor registry information may be shared with an out-of-state federally designated organ procurement organization that has been designated by the United States Department of Health and Human Services to serve an area outside our service area, in addition to eye banks and/or tissue recovery agencies.  Your donor registry email or mailing address may occasionally be used by the YesUtah or YesIdaho Donor Registry or DonorConnect to notify you of changes, news, developments, or events affecting the donor community.
    1. 3.2.     Information of Financial Contributors: Personal information provided in connection with a financial contribution will only be used for the specific purpose for which it was submitted.  
    1. 3.3.     Aggregate Information: Personal information may be used to generate aggregated data that does not allow the individual to be personally identified or contacted. Aggregated data shows visitor activity as a whole rather than on an individual basis and may be used to improve the effectiveness of the Registry and site. 
    1. 3.4.     Service Providers: Third parties may be used to provide services such as credit card processing, website content management, and email management. In all cases, DonorConnect ensures that third-party service providers maintain the privacy of your information in accordance with this Privacy Statement.
  4. Legal Requirements
    1. 4.1.     Other than as outlined above, personal information will only be released to third parties if a law, regulation, search warrant, subpoena or court order legally requires such disclosure. When we believe disclosure is appropriate, we may disclose Personally-Identifying Information in connection with efforts to investigate, prevent or take other action regarding illegal activity, suspected fraud or other wrongdoing; to protect and defend the rights, property or safety of Company, our users, our employees or others.
  5. Security
    1. 5.1.     Appropriate security measures (including encryption and other electronic and procedural safeguards) are used in order to prevent the loss, misuse, and alteration of personal information under DonorConnect’s control. However, no transmission over the Internet can be guaranteed to be completely secure. Consequently, DonorConnect and its service providers and their respective officers, directors, employees, and agents do not represent, warrant or guarantee that personal information will be protected against misuse, loss, or alterations.
  6. Correcting/Updating/Deactivating Personal Information
    1. 6.1.     Once an account has been created in the Registry, registered donors may access their personal profile online at any time to change or modify their information. If your personal information changes (such as postal address or email), or if you no longer wish to participate in the Registry, you may correct, update or deactivate your information via the YesUtah or YesIdaho Donor Registry websites (yesutah.org or yesidaho.org).     1. 7.1.     The YesUtah and YesIdaho websites may provide links to other third-party sites which may have different privacy policies. DonorConnect is not responsible for the privacy practices of such sites. It is strongly encouraged you read carefully and fully understand the privacy policies of any website collecting personally identifiable information.
  8. Notification of Changes
    1. 8.1.     Any changes to the  Privacy Statements for the YesUtah or YesIdaho websites will be posted on the respective sites so that users will be informed of what information is collected, how it is utilized, and under what circumstances (if any), it is disclosed. Information will be used in accordance with the Privacy Statement under which it was collected.
